Isn't it a bit pointless?
You've got to fire the managers who hired someone incapable of following a checklist, managers who didn't made sure it wasn't possible to get the wrong checklist, managers who made such failure uncheckable, or upper management who made it so that people below have to follow checklists and are incapable of noticing discrepancies.

BTW, both Filin and Remishevsky received not too favourable reports on their magagement skills from the people who worked under them or knew them. Filin, on the other hand, was an experienced technical specialist.
