...Itching during a TORU docking.
...Forgetting to put on earplugs (?) before going to sleep

...Headbanging during the first 72 hours in space.
...Activate both rendezvous and landing radar on a Apollo LM.

...Duct taping the ripped solar panel
...Drinking beer before spacewalk
 
I'd say the biggest survivable goof would be to accidentally abort your mission For example, hitting the abort button just as you are about to touch down on the moon or trigering the LES during lift-off.:rofl: You'd more than likely survive the event but you'd have some tough questions to answer for some very pissed off people on the ground!

The list of screwups you can make in space is pretty short, it's not exactly a forgiving environment. Forget to put your suit gloves on, and your EVA partner cycles the airlock and kills you because his field of view through the helmet is too narrow to see you dying of explosive decompression right behind him.
 
Accidentaly docking a DGIV with the nose cone closed, and then forgetting it's destroyed during the re-entry.
 
...sleeping in an airlock, before an EVA, that isn't yours
...going outside for a walk, and forgetting your EVA suit
...trying to open a window to let some fresh air in

Edit: It's more fun to think of unsurvivable ones, plus there are tons more possibilities.
 
Sitting in the Apollo CM, having to go to the bathroom, crank down the window and start looking for a gas station.

Explosive decompression is a myth. Nevermind the fact the suit could keep you alive by pumping more air in to replace the air moving out. The gas cloud would tip off Mr. Impatient that there is a problem too anyway.

The topic is the worst goofs that one can make and stay alive, so here is one from me: Ripping a waste bag. I think it might have happened in real life and it is one of the worst things I can imagine. A Apollo CM is about the size of a couple of phone booths and the only place you can run is a even smaller space that. And you do have to back in there to get home. Makes me gag thinking about it.
